How Are Feedlot Hospital Pens Designed for Sick Cattle?
By Jeremy Mills, CEO & Founder, Apex Grid Engineering — USAF Veteran. · Updated 2026-09-15
Hospital pens are engineered as treatment facilities: sited near the processing barn but separated from healthy pens, sized for individual and small-group cases, fitted with shade, soft footing, and night lighting, and connected to restraint so treatment is fast and low-stress.

Location, Separation, and Pen Sizing
The engineer sites the hospital area using three criteria: proximity to the processing barn and vet work area, separation from the healthy herd, and drainage independence. Adjacency to processing minimizes the daily movement of sick animals; separation — distance, wind direction, and drainage — protects the healthy pens from whatever the hospital pens are treating. The engineer checks the site grading plan to confirm hospital runoff does not sheet-flow into home pens, and details the pen floors to drain toward the runoff collection, not toward their neighbors. Sizing comes from the yard's pull rate: the percentage of animals pulled for treatment on a normal day, plus surge capacity for a respiratory outbreak or a bad weather event. The engineer provides individual pens for acute, down, or highly infectious cases and small group pens for animals on multi-day treatment that can safely commingle. Under-sizing the hospital area is a common and costly mistake — when it fills, sick animals stay in home pens, treatment slows, and the outbreak the hospital was built to contain keeps spreading.
Footing, Shade, Water, and Treatment Access
Comfort is treatment. The engineer specifies softer, drier footing for hospital pens than for home pens — well-drained material, sometimes bedding — because a sick animal that will not lie down comfortably will not recover well. Shade covers part or all of each pen so recovering cattle escape heat stress, and the shade structures are built to the same wind and snow standards as the rest of the yard. Waterers are placed for easy access by weakened animals, kept scrupulously clean, and detailed so the crew can drain and scrub them between occupants. Treatment access is drawn into the plan. Each hospital pen connects by a short, straight alley to a restraint chute or directly to the processing barn, so an animal can move from pen to treatment and back with minimal handling. Night lighting over the pens and the chute lets the crew pull and treat after dark, when much hospital work happens. The engineer also provides a small covered work area with water, power, and secure drug storage at the hospital pens themselves — the treatment crew's efficiency depends on having everything within steps, not across the yard.
Feedlot Hospital Pen Design Checklist
Use this checklist before hospital pens are built: • Sited near the processing barn and vet area, separated from healthy pens by distance, wind, and drainage • Individual pens for acute cases plus small group pens for multi-day treatment protocols • Pen count matched to the yard's pull rate with surge capacity for disease events • Shade over pens; soft, dry, well-drained footing that encourages resting • Waterers at accessible height, easy to drain and scrub between occupants • Direct alley or chute connection to the processing barn for low-stress treatment moves • Night lighting over pens and treatment areas for after-dark work • Hose bibs, washdown water, power, and secure drug storage at the hospital area • Dedicated equipment and traffic patterns keeping hospital work out of clean areas
